This form is intended to help you report potential violations related to utility pole attachments in New York State, as outlined under New York Public Service Law § 119-e.
By submitting this form, you are helping to identify potential safety hazards, non-compliant installations, and other concerns that may affect public safety, service reliability, or infrastructure standards. Please provide as much detail as possible regarding the location and nature of the issue so it can be reviewed and addressed appropriately.
PLEASE NOTE: This form should not be used for:
- Emergency situations (please dial 911)
- Concerns about “double poles” that do not constitute a safety hazard.
- Consumer complaints regarding utility bills, rates, or service-related issues. Such complaints may be submitted through the Department of Public Service complaint portal: https://dps.ny.gov/file-complaint.
Department of Public Service (Department) staff will review submitted reports and may conduct a desk audit and/or field inspection. If potential violations or safety hazards are identified, Department staff will notify the relevant pole owners and/or attaching entities for further evaluation and appropriate remediation, where necessary.
